Rehabilitation Wellness Home Physical Therapy offers outpatient physical and occupational therapy services delivered in the comfort of your home. This model ensures personalized treatment, continuity of care, and convenience—especially for patients with mobility or transportation challenges

Patients requiring physical or occupational therapy who prefer in-home sessions may be eligible. Eligibility is determined based on medical necessity.

Enrollment can be initiated through a physician’s referral or by contacting our office directly.

Appointments are available Monday through Friday, with both morning and afternoon sessions offered to accommodate your schedule.

Yes. Appointments may be rescheduled or canceled with at least 48 hours’ notice to avoid disruptions in care.

Services are typically covered by Part B benefits. Private pay options are also available. We recommend verifying coverage directly with your insurance provider.
